Care Memo™ is an app that helps individuals and family members monitor and manage a loved one’s health. Designed with privacy in mind, this invitation-only app grants access only to caregivers who are invited by a healthcare professional or someone they know. Approved users then get access to care team updates regarding their loved one and the ability to contact the team directly.

Care Memo also has a clinician-facing element that allows them to communicate with their patients and family members about care plan progress and goals with minimal change to their workflow.
